What is Ultima-Clomid by Ultima Pharmaceuticals?

Ultima-Clomid is a clomiphene citrate preparation from Ultima Pharmaceuticals, packed as 50 pills of 50 mg. Clomiphene is a selective estrogen receptor modulator: a molecule that blocks estrogen in some tissues and imitates it weakly in others, depending on the receptor and the cell type.

In the brain it behaves as a blocker. Estrogen normally signals the hypothalamus that hormone levels are high enough, and the hypothalamus then holds back gonadotropin-releasing hormone. When clomiphene sits on those receptors, the signal is lost, GnRH output climbs, and the pituitary answers with more luteinizing hormone and follicle-stimulating hormone. The gonads respond by producing more sex hormones and, in women, by maturing a follicle.

Clomiphene or tamoxifen?

Usual dosing ranges

For post cycle recovery, 25 to 50 mg once a day for three to five weeks is the range most often described, sometimes with a short period at 100 mg at the start. The off-label approach to low testosterone in men tends to use smaller, steadier doses over a longer time. In women, the standard fertility course is 50 mg daily for five days per menstrual cycle. Clomiphene is eliminated slowly, with a half-life of around five days, so blood levels keep rising through the first weeks of daily use.

What are the risks?

The side effect men talk about most is emotional: irritability, tearfulness or a short temper, which usually settle at a lower dose. Hot flushes, headache, nausea and breast tenderness are also common. Visual disturbances, including blurred vision and light flashes, are uncommon but require stopping the drug at once and an eye examination. Women can develop ovarian hyperstimulation and are more likely to conceive twins.

Pregnancy, liver disease, unexplained uterine bleeding, ovarian cysts not linked to PCOS, pituitary tumors and uncontrolled thyroid or adrenal disease are contraindications.
