What is Maxi-Fen-20 by Maxtreme?
Twenty milligrams is the standard daily dose of tamoxifen in breast cancer treatment, and that is the strength of every pill of Maxi-Fen-20. The product is made by Maxtreme and comes in a large pack of 100 pills of tamoxifen citrate (Nolvadex), enough for long treatment or several post cycle courses.
Chemically, tamoxifen is a nonsteroidal triphenylethylene, a relative of clomiphene. Pharmacologically it is a SERM: an estrogen receptor ligand that blocks estrogen in some tissues and partly mimics it in others. It is largely converted in the liver into more active metabolites, above all endoxifen, and both the parent drug and its metabolites leave the body slowly, with half-lives counted in days rather than hours.
Effects and purpose
In oncology, tamoxifen starves estrogen-dependent breast tumors of the hormone signal they need to grow. That same receptor blockade at the breast is what athletes rely on when estrogen from aromatizing steroids starts to affect the nipple area. Maxi-Fen-20 can calm sore, swollen nipples and stop gynecomastia in its early phase.
The second purpose is hormonal recovery. At the hypothalamus, tamoxifen prevents estrogen from signaling that hormone levels are sufficient, so the pituitary produces more LH and FSH, and the testes respond with more testosterone and sperm. That is why Maxi-Fen-20 is one of the two classic SERMs of post cycle therapy, used either on its own or together with clomiphene. Unlike aromatase inhibitors, it does not lower estradiol, so the benefits estrogen has for joints, lipids and mood remain.
Dosing guidelines from practice
Breast cancer therapy uses 20 mg daily, sometimes 40 mg, for five years or more. Commonly reported post cycle schemes start with 40 mg a day for one or two weeks and continue with 20 mg a day for two to four more weeks, beginning once the last steroid has cleared. For early gynecomastia during a cycle, 20 mg a day is the figure seen most often. Since tamoxifen reaches stable levels only after a few weeks, a higher first dose is used to speed things up.
Exact amounts and duration must be discussed and agreed with a qualified medical professional, who can also check hormone levels before and after the course.
Side effects
- Hot flushes, sweating and nausea.
- Mood swings, tiredness and reduced libido in some men.
- Blood clots, such as deep vein thrombosis or pulmonary embolism, and a higher risk of stroke.
- Eye problems, including cataracts and retinal changes.
- Changes in liver enzymes and fatty liver with long use.
- In women, thickening of the uterine lining and a higher risk of uterine cancer.
Tamoxifen also lowers IGF-1 somewhat. It strengthens the effect of warfarin, reduces blood levels of letrozole and anastrozole, and should not be used in pregnancy or by people with a past blood clot.
